By completing and submitting the form attached, I understand that in the event that any services are/were provided today are solely for the purpose of education, relaxation, and/or relief of muscular tension. Services include but are not limited to; manual muscle therapy, massage therapy, instrument assisted soft tissue mobilization, kinesiology taping, corrective exercises, or Normatec Compression Boot Therapy. If I begin to experience any pain or discomfort during my session, I will immediately inform the provider or therapist. Furthermore, I understand that this session is not a substitute for a medical exam, diagnosis or treatment of any condition that would normally be provided by a physician, massage therapist, or any other qualified professional medical specialist. Because services rendered during this session should not be performed on those who have certain medical circumstances, I hereby affirm that it is my duty to inform the provider of any major medical conditions that I have in writing before services are rendered. I agree that there will be no liability to the provider should I fail to uphold any portion of this agreement, should any harm be done during this session, or should I fail to provide medical updates during the session.
